Mandibular Sagittal Split Osteotomy Mandibular sagittal split osteotomy, also known as bilateral sagittal split osteotomy or BSSO, is an advanced orthognathic surgery procedure used to correct lower jaw position, facial imbalance, and bite problems related to mandibular deficiency, mandibular excess, asymmetry, or malocclusion. The procedure is performed under general anesthesia in a fully equipped hospital. Through incisions placed inside the mouth, the lower jaw bone is carefully divided in a controlled sagittal plane. The mandible can then be moved forward, backward, or adjusted asymmetrically according to the patient’s functional and aesthetic needs. The new jaw position is fixed with titanium plates and screws, with no visible external scar. Treatment planning usually requires clinical examination, dental and orthodontic assessment, facial analysis, photographs, cephalometric X-rays, and when necessary, 3D CT evaluation. In many patients, mandibular sagittal split osteotomy is planned together with orthodontic treatment, and in selected cases it may be combined with upper jaw surgery or genioplasty. The aim of surgery is not only to improve facial harmony, but also to correct the dental occlusion, improve chewing function, and create a stable jaw relationship. Recovery includes swelling during the first weeks, a soft diet period, oral hygiene care, and regular follow-up visits.
