How Do Braces Work?

In their entirety, braces work by applying continuous pressure over a period of time to slowly move teeth in a specific direction. As the teeth move, the bone changes shape as pressure is applied.

Braces are made up of the following components:

  • Brackets are the small squares that are bonded directly to the front of each tooth with a special dental bonding agent or are attached to orthodontic bands. Brackets act like handles, holding the arch wires that move the teeth. There are several types of brackets, including stainless steel and tooth-colored ceramic or plastic, which are often selected because they’re less obvious. Occasionally, brackets are cemented to the back of teeth, in order to hide them from view.
  • Orthodontic bands are stainless steel, clear, or tooth-colored materials that are cemented to the teeth with dental bonding agents. They wrap around each tooth to provide an anchor for the brackets. The clear or tooth-colored bands are more cosmetically appealing options but are more expensive than stainless steel. They are not used in all patients. Some people have only brackets and no bands.
  • Spacers are separators that fit between teeth to create a small space prior to placement of orthodontic bands.
  • Arch wires attach to the brackets and act as tracks to guide the movement of the teeth. Arch wires can be made of metal or be clear or tooth-colored.
  • Ties are small rubber rings or fine wires that fasten the arch wire to the brackets. They can be clear, metal, or colored.
  • A buccal tube on the band of the last tooth holds the end of the arch wire securely in place.
  • Tiny elastic rubber bands, called ligatures, hold the arch wires to the brackets.
  • Springs may be placed on the arch wires between brackets to push, pull, open, or close the spaces between teeth.

Fixed Braces
A fixed braces or fixed appliance is a dental appliance which cannot be removed by the patient. Dental appliances are used to move the teeth to correct , situations in which the teeth are not aligned properly in the mouth. Depending on the nature of the , a dental appliance may be worn primarily for cosmetic reasons, to straighten teeth which look unsightly, or it may be important for medical reasons, to correct a malocclusion which may cause difficulty eating, increase the risk of oral infections, or cause other problems. Fixed appliance therapy also requires a commitment on the part of the patient to make regular appointments and to take care of the appliance. If the appliance is left in the mouth without adjustments, it can potentially cause damage to the mouth and teeth.
 
Lingual Braces
Lingual braces are the most invisible type of dental braces of them all! What makes lingual braces so unique is their placement: Unlike any other type of dental braces, lingual braces are placed behind your teeth. With lingual braces, you don't have to worry about hiding a "metal mouth"; you can smile like you've got a secret. Lingual braces are not suitable for adults and teenagers who have small teeth or excessive bite problems (particularly an ). In general, lingual braces cost more than traditional braces because the brackets are custom-made. Some patients may also find it more difficult to adjust to lingual braces because the positioning affects the tongue, making swallowing and talking a little tricky.

The Procedure and After-Care

Your first appointment for lingual braces involves taking an impression of your teeth. Orthodontic Retainer

In order to prevent the teeth moving back to their original position, may be worn once the treatment with braces is complete for the patient depending on their specific needs. Retainers help in maintaining and stabilizing the position of teeth long enough to permit reorganization of the supporting structures after active phase of orthodontic therapy. If the patient does not wear the braces appropriately for the right amount of time, the teeth may move towards their previous position. For regular traditional braces Hawley retainers are used. They are made of metal hooks that surround the teeth and are enclosed by an acrylic plate shaped to fit the patient's palate. For invisalign braces an Essix retainer is used. They are similar to the regular invisalign braces and is a clear plastic tray that is firmly fitted to the teeth that stays in place, and does not have a plate fitted to the palate. Orthodontics is a branch of dentistry that specializes in treating patients with improper positioning of teeth when the mouth is closed (malocclusion), which results in an improper bite. Dental bleaching, also known as tooth whitening, is a common procedure in general dentistry but most especially in the field of cosmetic dentistry

Whitening is not a one-time procedure. It will need to be repeated from time to time if you want to maintain the brighter color.

What It's Used For

The outer layer of a tooth is called the . The color of natural teeth is created by the reflection and scattering of light off the enamel, combined with the color of the under it. Your genes affect the thickness and smoothness of the enamel. Thinner enamel allows more of the color of the dentin to show through. Having smoother or rougher enamel also affects the reflection of light and therefore the color.

Every day, a thin coating (pellicle) forms on the enamel and picks up stains. Tooth enamel also contains pores that can hold stains.

The most common reasons for teeth to get yellow or stained are:

  • Using tobacco
  • Drinking dark-colored liquids such as coffee, cola, tea and red wine
  • Not taking good care of your teeth

Aging also makes teeth less bright as the enamel gets thinner and the dentin becomes darker.

It is also possible to have stains inside the tooth. These are called intrinsic stains. For example, intrinsic stains can be caused by exposure to too much as a child while teeth are developing. Other causes include antibiotics. They can stain a child's teeth if taken by a mother during the second half of pregnancy or by a child who is 8 years old or younger. Teeth are still developing during these years. Trauma may also darken a tooth.

Tooth whitening is most effective on surface (extrinsic) stains.

Chemical Teeth Whitening

Most whiteners use a peroxide-based chemistry. Peroxide compounds have the ability to whiten teeth through an oxidation process. It has been demonstrated that peroxide has the ability to penetrate the inner aspect of the enamel within a matter of only a few minutes after application. There are complex organic molecules within the tooth that reflect a specific wavelength of light and are responsible for the color. It is believed that by releasing free radicals, peroxide has the ability to oxidize the color-producing organic compounds found within the enamel’s matrix. Laser Teeth Whitening

Laser Teeth whitening can be performed with or without light technology. Light technology is referred to as photopolymerization and was first approved by the FDA for teeth whitening in 1996. There are two light techniques that can be used: blue-light or argon laser light. Both techniques require about one hour in the dental chair. Application of a high concentration hydrogen peroxide is applied to the tooth surface four times at 15-minute intervals. Both light systems claim to work by accelerating the chemical reaction. A whitening agent is placed in the trays and they are worn usually once a day for up to three hours. The length of time worn and duration is determined on factors considered by you and your dentist. Custom trays are made to continue the whitening process at home and ensure you achieve optimum whitening results.

The primary benefit of power whitening is that the process takes only one in-office visit and results can be dramatic and almost immediate. The disadvantages include cost. Power whitening is significantly more costly than other methods. You are paying for the technology, professional application and guidance along with the convenience of fast results.
