Klinik Cybermedik pada tertubuh pada tahun 2004. Pada asalnya ia terdiri daripada 2 buah klinik yang berbeza nama iaitu Klinik Nik Isahak Cawangan Sek 34 Shah Alam dan Poliklinik Iman. Kedua-dua klinik ini diasas kan oleh Dr Azman Mohamed Sharif semenjak 1997 dan 2001 lagi.
