Eyebrow Transplant

Transplantation to the eyebrows is a procedure designed to restore growing hair to eyebrows that are overly thin, scarred, or completely missing. The absence of hair can be due to genetics, prior electrolysis or laser hair removal, overplucking, thyroid or other hormonal abnormalities, or trauma due to surgery, burns or other types of accidents. The donor hairs come from the scalp which, when transplanted into the eyebrows, continue to grow for a lifetime and therefore need to be trimmed typically once a month. To provide a natural appearance, the hairs are transplanted primarily one and occasionally two at a time, the natural way eyebrow hairs grow. This is a very delicate procedure, requiring perfect placement of these hairs into tiny (half-millimeter) incisions that are angled at just the right direction and positioned to mimic natural growth. The use of all-microscopically dissected [grafts] allows their placement into the smallest possible incisions so as to minimize scarring and damage to already existing hairs.

A procedure typically involves the placement of 50 to as many as 325 hairs into each eyebrow, depending upon the existing amount of hair and the desired size and density. Performed usually under a mild oral sedative, the 2 hour procedure is essentially painless, as is the recovery period. For the first 2 to 4 days after the procedure, tiny crusts are around each transplanted hair. By 3 to 5 days, other than some occasional mild pinkness which fades out by the first week, patients are able to return to normal activities without any sign of having had a procedure. Sutures that are placed in the donor area are removed at 1 week. The transplanted hairs fall out at around 2 weeks, then start to regrow at 3 months, where they will continue to grow for a lifetime

Eyelash Transplant

Eyelash hair transplantation, also known as eyelash implants, is a relatively new procedure that originally was used to treat areas of the eyelid that have no eyelashes due to genetics, alopecia, or trauma such as that from cancer surgery or burns. Recently, however, Dr.RVB has been using the procedure to thicken and enhance the appearance of the eyelashes, in individuals who already have eyelashes.

The donor hairs come from the scalp which then become eyelashes and they continue to grow for a lifetime and therefore need to be trimmed typically once a month. To provide a natural appearance, the hairs are transplanted one at a time. After the patient’s scalp is anesthetized, the doctor removes a strip of tissue from the donor area, an area within the permanent zone, and, once extracted, the donor strip is cut into many individual follicular units using microscopic dissection techniques. As these follicular unit grafts are being prepared, the doctor makes tiny holes in the scalp, called recipient sites, where the grafts are placed.

The arrangement and positioning of these follicular unit grafts determines the aesthetic qualities of a hair transplant, and so this arrangement must be decided on a case-by-case basis, depending on the patients’ history of hair loss and likelihood of future hair loss. FUE - Follicular Unit Extraction

Follicular Unit Extraction (FUE) is a method of extracting, or “harvesting,” donor hair in a follicular unit hair transplant procedure. In FUE, an instrument is used to make a small, circular incision in the skin around a follicular unit, separating it from the surrounding tissue. The unit is then extracted (pulled) directly from the scalp, leaving a small open hole.

This process is repeated until the hair transplant surgeon has harvested enough follicular units for the planned hair restoration. This process can take one or more hours and in large sessions, may be accomplished over two consecutive days. The donor wounds, approximately 1-mm in size, completely heal over the course of seven to ten days, leaving tiny white scars buried in the hair in the back and sides of the scalp.

This method of donor harvesting, removing follicular units one-by-one directly from the scalp, is what differentiates the FUE procedure from a traditional Follicular Unit Transplant (FUT), where the donor hair is removed from the scalp in one thin, long strip and then subsequently dissected into individual follicular units using a stereo-microscope.

Before the grafts are harvested, tiny “recipient sites” are made in the balding area of the scalp using a fine needle-point instrument. The follicular units are then placed into the recipient sites where they will grow into healthy hair-producing follicles. The creation of recipient sites and the placing of follicular unit grafts are essentially the same in both FUE and FUT procedures. The difference lies in the appearance of the donor area and in the quality and quantity of grafts obtained with each technique.

PRP in Hair Transplantation

The potential for using PRP to promote healing and hair growth after hair transplantation is centered in three functional applications:

  1. To preserve and enhance hair follicle viability during and after hair transplantation;
  2. To promote and enhance tissue repair and healing after hair transplantation; and,
  3. To reinvigorate dormant hair follicles and stimulate new hair growth.

To Preserve and Enhance Hair Follicle Viability

Between the time that hair follicles are removed from a donor area of the scalp and transplanted into a recipient area, they are subject to damage from several causes:

  • Dehydration if the donor follicles are inadequately moistened between the times of removal and transplantation;
  • Oxygen and nutrient starvation due to being removed from blood supply during the harvest-to-transplantation time period;
  • Temperature and acid/alkaline changes in the follicle environment; and,
  • Revascularization injury when the donor follicles are transplanted to the recipient site and must re-adapt to having a blood supply.

A common approach to maintaining donor hair follicle viability during the transition period is to keep them in a storage solution that provides a protective environment of appropriate temperature, chemical balance and nutrient supply. Recent research has indicated that addition of PRP to the storage solution improves follicle viability during and after transplantation, enhances post-transplantation tissue healing and promotes hair growth in transplanted follicles. An approach advocated by some investigators is to bathe the donor hair follicles in activated PRP just prior to transplantation.

Investigators have reported that PRP promotes hair growth from follicles by the action of platelet growth factors on hair follicle stem cells
