Sometimes, if there is too muchskin that folds down from the upper eyelids, it may cause the eyelash to gointo the eye, in a condition called entropion.   In all these conditions, relief is found inan upper eyelid blepharoplasty. In the lower eyelid, the extrabulge just below the eyelashes is more likely due to the hypertrophy of themuscle and not because of fat. However, if you find the bulge to be about ahalf centimeter or a centimeter below the eyelashes, the bulge is more likelydue to the fat that is pushing out the skin. The drooping of the upper eyelidstogether with the bulging of the skin and fat at the lower eyelids give anindividual the “tired look” and “adds age” to the patient’s facial features.  Removal of these excess fat and skin, both inthe upper and lower eyelids gives the individual a more “refreshed” look of theface There are patients who, becauseof the chronic presence of bulging of fat on the lower eyelids has caused someweakness of the canthal ligaments; which hold the margin of the lower eyelidsclose to the eye. In these group of patients, an additional procedure calledcanthopexy prevents the white of the eye from being overexposed.
