Dr. Anand N S, General & Laparoscopic surgeon, Beams Hospitals,Koramangala Bangalore performed this procedure on a 46 Year old housewife was suffering from Severe Rheumatoid arthritis for a long duration. She was seen in BEAMS hospitals Koramangala Bangalore OPD for swelling in the umbilicus (Large umbilical hernia). On evaluation she gave a history of menorrhagia for long duration she also gave history of mass per abdomen (Was diagnosed to have fibroids in 2004, but patient was reluctant to undergo treatment). She was found to have a large uterus (about 7-8 months pregnant uterus size with more lateral extension). She was anaemic about 6g Hemoglobin (Normal 12-14) and was also found to be diabetic.
She was started on oral hematinics to increase hemoglobin and advised diabetes control.
Patient was planned for Laparoscopic Total hysterectomy and repair of hernia.
Challenges in the surgery:
1. Large uterus occupying almost the entire abdomen and abutting on the important surrounding structures which can be damaged during the process.
2. Conversion to open as the visibility may be hampered due to large size, bleeding and removal of such big uterus from abdomen through such small hole.
3. Severe rheumatoid arthritis with deformities in the upper limb and neck-- difficulty in anesthesia and positioning of the patient for surgery.
4. Diabetes mellitus.
Initially the hernia was reduced, that is the abdomen fat (Omentum) stuck in hernia was removed. The Uterus had compressed the urinary bladder and had pushed the appendix and the large intestine up. The uterus was separated from one of the large fibroids attached to it and later disconnected from the blood vessels and other connections. It was removed using special equipment called Morcellator. The weight of the uterus was 2.5 kg. The hernia was repaired using a mesh.
Post Surgery: She recovered well and was given liquids the next day. She started walking independently and taking regular food from the next day. She was discharged on the fourth day of surgery.
The patient who underwent the surgery says, "I was in pain and discomfort with severe bleeding during periods which lasted almost 45 days. In the past four years, I had visited several doctors in the city; they could not convincingly give me a total solution for my problem. Few recommended only hernia repair and few open surgery for removal of uterus.
Finally I approached Beams hospitals whose team of doctors studied my case carefully and then recommended me to go for the laparoscopy surgery and treat both issue of fibroids and hernia together. I was very nervous initially due to bad experiences with few medical facilities, so had postponed the surgery a couple of times. Finally when it was done the surgery went on for eight hours. Thanks to the competent and professional team at Beams Hospitals, I walked on the same day of surgery and started eating the next day. I can now lead a normal life once again."
Patient's husband mentions that previous bad experience with some medical facilities lead us to a fear psychosis and seeing a well-coordinated effort in Beams Team was a rare observation.
