Breast Augmentation
Breast augmentation is usually accomplished through the use of a silicone implant placed behind the breast tissue. The implant may be placed behind the chest muscle (sub-pectoral) or in front of the muscle (sub-glandular), depending upon the patient’s individual breast shape. Insertion incision can be under the arm, around the areola (pigmented nipple border) or under the lower breast border (inframammary fold). There are two general types of silicone breast implants: saline-filled and gel-filled. Saline-filled implants are can be placed through smaller incisions, are less stimulating of surrounding scar, and are easier to replace. However, they are slightly more “wrinkly” feeling at the lower edge of the breast (inframammary fold). Gel-filled implants feel slightly more natural to touch, but require a slightly larger incision, are historically more likely to stimulate internal scar tightening, and are somewhat more complicated to replace.

Traditionally, facial aging has been considered as only loose skin. And facelift procedures were aimed at skin re-draping and removal. While this simple procedure is still very common, pulling on the skin inevitably eventually results in the “stretched,” “wind-blown” appearance that people fear. It is my strong opinion that the proper way to surgically rejuvenate the face is to reposition all the layers of the face as a unit, using the deeper fibrous support layers – never pulling on the skin. The variation of this approach that I developed has come to be known as the “High SMAS” facelift technique. After repositioning, redundant skin is removed in place. For over 25 years, this procedure has produced natural, long-lasting results. The procedure can be performed in our surgicenter. An overnight stay is recommended. While full social appearance is limited for six weeks, normal activities can be resumed in 10 to 14 days.

As the result of trauma (often unrecognized at the time), the internal and external structures can be damaged and displaced. Displacement of the internal partition (deviated nasal septum) may affect breathing as well as cause the nose to be externally deflected to one side. Surgical procedures can help open the airway and restore alignment of the nose. The external shape of the non-injured nose is genetically determined. The nose is actually a collection of separate small tile-like structures of bone and cartilage. These “skeletal” elements impart shape to the overlying skin, similar to tent poles shaping a tent. Cosmetic rhinoplasty involves re-shaping and repositioning the bone and cartilage tiles, allowing the overlying skin to re-drape. Skin incisions are usually minimal. An external splint is usually needed for a few days.
 
Abdominoplasty
After pregnancy or significant weight loss, the abdominal skin may remain loose, particularly in the lower abdomen. There is no non-surgical treatment which will significantly tighten loose skin. Using an incision low in the abdomen and in the body crease, excess skin can be removed. In limited lower abdominal looseness, a modified (“mini”) abdominoplasty can be performed. This procedure uses only the lower incision. If loose skin is present above the navel, a full advancement is needed. In such cases, an additional incision around the navel is required.
