Open Brow Lift
The open brow lift uses an incision hidden in the hair-bearing scalp to lift and redrape the brow and the forehead. The open brow lift remains a relevant and effective technique for its ability to predictably and reliably reposition the eyebrow and redrape the forehead. This technique is particularly useful for patients with very descended brow, with significant excess skin and thick and heavy forehead skin.
 
Endoscopic or Limited Incision Brow Lift
The endoscopic brow lift is becoming the most popular technique in brow lifting. Because of the precise release and visualization possible with the endoscope, incisions can be kept to a minimum and as inconspicuous as possible

This technique is ideal for patients with early brow descent, with significant descent of the brow but not much skin laxity.
 
Deep Temporal Lift
This short incision brow lift is very useful and effective for patients with descent of the tail of the brow. Dissection required is minimized and recovery rapid.

The incision also allows for a simultaneous access to the midcheek. Precise release of the midcheek retaining ligaments allow the midcheek to be lifted and an effeective midcheek lift be performed together with the brow lift without needing additional incisions.

Dimple creation is a simple surgical procedure that aims to recreate this naturally occurring anatomy. The ideal position of the dimples varies from patient to patient. The decision as to where to place the dimple is made with the patient’s input and preference at the time of the procedure.

PTOSIS Correction
Congenital ptosis results from congenital deficiency of the muscle that opens the eyes, resulting in inability to fully open the eyes, it can be surgically corrected.

More common is the condition called senile ptosis. With aging, the levator of the upper eyelids that opens the eyes gradually weakens and eventually tears (Levator dehiscence). This result in difficulty with opening the eyes as well as subconscious use of the forehead muscles (which raises the eyebrow and causes transverse forehead wrinkles) to assist in opening the eyes. Senile Ptosis results in inability to completely open the eyes, resulting in blocked vision as well as straining of muscles around the eyes. The patients complain of being unable to see well as well as feeling tired from straining in an attempt to open the eyes. A surgical procedure called levator advancement is used to repair the torn (or dehisced) levator aponeurosis of the upper eyelids.
 
Ectropion / Lower Eyelid Malposition Correction
Ectropion is a condition where the lower eyelid everts and gets pulled away from the eyeball. Lower lid malposition is a condition where the lower eyelid is pulled lower, resulting in visibility of the inferior rim of the pupil.

Ectropion of the right lower eyelid as a complication from a previous lower eyelid surgery. Ectropion and lower eyelid malposition can occur in isolation or associated with each other. Ectropion and lower eyelid malposition can be the result of laxity as a result of aging or more commonly as a result of previous surgery.

Correction is complex and requires a combination of techniques, depending of the specific needs of the patients. This can range from tightening the lower eyelid, to the need to lift the midcheek to recruit more skin into the deficient lower eyelid or even the need to the use of grafts from the ear or the palate to address the deficient support of the lower eyelid.

Absent or Asymetric Upper Eyelid Crease
An absent upper eyelid crease or fold is commonly seen in ASIAN eyes. Upper eyelid crease creation or ‘Double eyelid’ surgery as it is commonly called, is a procedure designed to produce a crease in the upper eyelid. Upper eyelid fold creation has the profound optical illusion of making the eyes appear larger and brighter. The person appears more approachable and friendly as a result.
 
Suture Upper Eyelid Blepharoplasty
Also called the scarless upper eyelid creation, this technique involves precise placement of several key sutures into the upper eyelid to create a crisp fold in the upper eyelid. Technique and precision is important in executing this procedure. The sutures need to have sufficient holding power to form a crisp crease in a symmetrical position. The technique is versatile in that the height and shape of the fold can be precisely controlled by changing the placement of the sutures. Minor asymmetries of the upper eyelid, when present, can also corrected by placing the sutures in slightly differential positions in each upper eyelids. As the procedure does not involve cutting, down time is minimal and patients can return to work and resume their usual activities after a few days. This technique is suitable for patients without an upper eyelid crease but with no excess fat or puffiness.
 
Incision Upper Eyelid Blepharoplasty
This method, also called the upper eyelid blepharoplasty, involves the conservative excision of excess skin. The advantage of the incision method is that it offers tremendous versatility by allowing creation of precise upper eyelid crease and removal of true excess fat (to reduce the puffy upper eyelids that some patients complained of) when these are present. By fixing the skin directly to the upper lid structures below it, a very robust and sculpted upper eyelid crease can be reliably and predictably created, giving a crisp and well formed upper eyelid fold. Because of the versatility afforded by the incision technique, the fold can be exactly created to the suit each patient’s needs. Lost of fold is very rare with the incision technique.
Blepharoplasty
 
Caucasian Upper Eyelids
The aging Caucasian upper eyelid first presents as subtle loss of definition of the eyelid fold and ‘crepey’ skin above the eyes. This makes it a little more difficult to apply eye make-up evenly. With further aging, a look of heaviness and hooding develops, and eventually the upper eyelid fold becomes completely covered by the excess upper eyelid skin.

The internal fixation technique in contrast operates internally, on the support layer of the upper eyelid, and not directly on the skin itself. This is the key to natural and lasting results. It restores the pleasing contour and curve above the eyes and remove the heaviness and drooping that occurs with age. The skin above the lash is also directly tightened, and the lashes which droops slightly with aging are naturally returned to their more youthful position. Essentially, the technique surgically recreates the internal eyelid structure found in people with beautiful eyes.
 
Male Upper Eyelids
Male upper eyelid surgery is profoundly different from upper eyelid surgery in female patients. While the rationale for surgery, visual impression, emotional and aesthetic goals that the procedure strives to achieve remains fundamentally the same as upper eyelid surgery in the female patients are the same.

In general 2 group of patients benefit from male upper eyelid surgery:
1. Young patients with absent upper eyelid crease as in the patient in Figure. This would give the aesthetic benefit and change as we have discussed above.
2. Older patients, where aging has caused significant hooding of the upper eyelids. Upper eyelid surgery can brighten up the eyes and make the person look more alert and friendly.
 
Lower Eyelid (Eyebags)
Transconjuctival technique is very effective in patients with eye bags, without other problems of midcheek aging. As the procedure does not involve cutting the skin from the lower eyelid, this technique is also called scarless eye bag removal.
 
Composite Facelift
We can perform a composite facelift, an advanced surgical technique that focuses on restoring and tightening the deeper support layer of the face, which is referred to as superficial musculoaponeurotic layer (SMAS).

The procedure is called a Composite Facelift as the SMAS, subcutaneous fat and skin is lifted as a single unit (hence the term COMPOSITE FACELIFT), to achieve a natural and long lasting result. The SMAS layer is the natural support layer of the face, and this is where anatomically laxity develops. Tightening the SMAS layer therefore directly addresses and reverses the aging changes of the face, while producing a more natural and long lasting facial rejuvenation. This is done by restoring the shape and contours of youth to the cheek, jawline and neck.

The results of a composite facelift are very different from the traditional skin facelift, as it is natural with no telltale signs of surgery. As tightening is done at a deeper level, the face looks fresh without appearing excessively pulled. The youthful shape of the face is also more effectively restored with this approach.
 
Fat transfer/ Fat Grafting
Fat grafting or more precisely microstructural fat grating is a procedure where fat is aspirated or sucked out from one part of the body, usually the abdomen or hips and transferred to another area in need of volume enhancement, usually in the face, breast or buttocks. While fat grafting has been around for several decades, there has been a renaissance of sort in the application of fat grafting in the past few years. This can be attributed to the advancements in the science as well as the art of fat grafting in recent years, making the grafting more reliable and the results better.

Fat grafting is a highly operator or technique dependent procedure. At the W Clinic, we adhere strictly to the Coleman technique when performing fat grafting, currently the gold standard in fat grafting.

Liposuction is a procedure where subcutaneous fat (Fat under the skin) is removed by suctioning the fat out. Depending on your specific request, skin condition and the amount of fat needed to be removed the power assisted technique or ultrasonic assisted technique may be used. Liposuction may be used alone or as an adjunct to other procedures such as a tummy-tuck procedure.

Skeletal Augmentation
Skeletal augmentation can be done alone or more commonly with the surgical rejuvenation (known as the ‘lift and fill’ approach).

Skeletal augmentation can be done with a variety of materials. It can be done with facial implants such as silicone, medpore or more recently hydroxyapatite granules. Hydroxyapatite granules in particular have been gaining popularity recently. It has the same chemical composition as bone and integrates completely with the facial skeleton. It is very safe when used in the face.

The use of fillers (Hyaluronic Acid fillers) are also very effective with minimal down time. A variety of fillers are currently available for different areas of the face and they vary in flow characteristics and permanence. Fillers last from 9 months to 2 years depending of the material used.

Fat grafts may also be used. The benefit of the use of fat is that, once taken it is a permanent ‘filler’ material. Also, fat have the effect of rejuvenating the overlying skin when injected into the face. Fat may be harvested via the abdomen or the thighs via tiny incisions under local anesthetic with minimal discomfort.

Biography:
Dr Wong Chin Ho is a Plastic Surgeon accredited by the Ministry of Health of Singapore. He graduated from the Medical Faculty of the National University of Singapore in 2000 and was one of the top students of his graduating class. He completed his Basic Surgical Training in General Surgery in 2004 and went on to complete his Advance Surgical Training in Plastic Surgery at the Department of Plastic Reconstructive and Aesthetic Surgery at the Singapore general Hospital in 2008.

Dr Wong is fellowship trained in Aesthetic Plastic Surgery and Reconstructive Microsurgery. In 2007, under a scholarship by the Human Manpower Development Program administered by the Ministry of Health of Singapore, he went to do a 1-year Reconstructive Microsurgery and Asian Aesthetic Surgery fellowship under Professor Fu-Chan Wei at the Chang Gung Memorial Hospital in Taiwan. This was followed by a 6 month Advanced Facial Aesthetic Surgery Fellowship with Dr Bryan Mendelson in Melbourne Australia in 2009. In 2010, he was the first recipient of the International Fellowship awarded by the American Society for Aesthetic Surgery (ASAPS).
