When it comes to expressing your deepest thoughts and emotions, there is probably no facial feature quite as important as your eyes. When they look tired, saggy or puffy, more than just your appearance may suffer - the image you project to others is affected as well.

Drooping, overhung upper lids and sagging lower lids can happen to anyone. Eyelid problems often are one of the first signs of aging, but even young people can be affected. Chronic allergies can cause the eyelids to swell, speeding the development of overhanging upper lids or bags under the eyes. Sun exposure, excess squinting and habitual eye-rubbing can promote sagginess. Heredity also plays a part.

Fortunately, eyelid problems can be treated very effectively through blepharoplasty, a relatively simple procedure that removes excess skin, muscle and fatty tissue. It often is combined with other facial plastic surgery procedures, such as browlift, facelift or skin resurfacing.

Asian Blepharoplasty

Half of eastern Asian patients have what is referred to as a single lid. These patients may wish to have a more open look by creating a double lid (ie eyelid crease). Some patients with a double lid (eyelid crease) may also want surgery to make the crease higher. An important consideration of surgery is to make these changes while still preserving the elegant shape and appearance of the Asian eyelid. The aim is not to "westernize" the eye with a deep set crease.


In addition surgery can be offered for patients who may wish to eliminate the fold in the inner corner of the eye referred to as the epicanthal fold. This can be done as an isolated procedure or as part of the double eyelid procedure.


None of us can escape the ravages of time. Inevitably, gravity, personal habits, heredity and sun exposure can influence the aging process. As your skin begins losing its elasticity and muscles and other soft tissues weaken, jowls form along your jaw line. Smile lines become permanent, creases develop around your mouth and chin and vertical folds may appear at the front of the neck.

Facelift surgery refers to a procedure where the excess skin of the face is removed and a deeper layer of stronger tissue, referred to as the SMAS, is tightened. In certain cases, men may have a more limited procedure referred to as necklift. An important trend to note is the move towards having the procedures done at an earlier age. Not only does it seem to be that the benefits appear to last longer, the change in appearance is less pronounced. A typical facelift will usually result in an appearance 5 to 7 younger than your actual age.


Facial Liposuction

Fatty deposits under the chin can be managed with a procedure called submental liposuction. Liposuction reduces the number of fat cells in a localized area. It is ideal for refining the contour of the neck and jaw line and eliminating heavy jowls.

Anytime you have a fullness of the neck behind and below the chin despite being at a reasonable weight is the time to consider liposuction surgery. This can be performed as an isolated procedure usually up to ones early 40s. Beyond this age, it must be combined with a facelift as the skin loses its elasticity as we get older and therefore it may not redrape in a natural fashion unless combined with facelift surgery.

After liposuction a snug dressing is applied and usually removed by Dr Bartlett the day after surgery. Thereafter you wear an elasticized facial garment for approximately one week. Pain is minimal, most patients using mild pain relievers.

Normally, the ears begin to take shape early in pregnancy. Delicate structures of cartilage and soft tissue covered by a thin layer of skin, the ears first stick straight out. Then, as the natural folds of the ear develop, they gradually assume a position closer to the head. By the time of birth, a babys outer ears have achieved the complex arrangement of folds that give the ear its characteristic shape.

For unknown reasons in certain individuals, the normal folds may fail to develop with resultant cup-like ears that project from the side of the head. 

In other cases, excess ear cartilage develops, resulting in normally shaped ears that stick out too far. Occasionally, the ears just grow to be too large. Because each ear develops independently, it is not uncommon for one ear to protrude more than the other, or have a noticeably different shape leading to an unbalanced look.

As we age, the outer ear undergoes more changes. Collagen fibers relax with time and the ears of older people develop telltale creases and elongated earlobes.

You return home the day of the procedure but must wear a head dressing for 1 week . Thereafter, most patients wear a head band during sleep for another 4 – 6 weeks to lessen the tension on the ears as further healing takes place.

Lip enhancement usually refers to augmentation of the lip either upper , lower or both. Although there are surgical procedures which can be offered in select patients, the vast majority of lip enhancements may be achieved with a variety of fillers eg (temporary) or  (semi-permanent).

Because of the sensitivity of the lip, Dr Bartlett usually anesthetizes the area with a nerve block prior to using a filler to augment the lip. In addition to augmenting the lip, fine vertical lip lines can be addressed with a filler or alternatively may be managed with laser resurfacing.

Depending on your age, sun exposure, skin type and facial animation the choice of treatment(s) for facial wrinkles may vary. For fine lines related to facial animation such as frown lines, forehead lines and crows feet, treatment for lines and wrinkles may be the best treatment.

Laser resurfacing can be performed to manage finer lines of the lower eyelids which can form as one ages. Laser resurfacing is also effective at managing fine or deeper lip lines as well as generalized treatment of the entire face for diffuse texture change or multiple fine lines.

A Graduate of Queens University Medical School in Kingston, Ontario, Canada; Dr. Bartlett received his surgical training at the University of Washington and University of British Columbia. He has pursued further post specialty training in Facial Plastic and Reconstructive Surgery in Switzerland, Italy and the United States.

Dr. Bartlett is one of the few surgeons in the Vancouver area to have completed a Fellowship with the American Academy of Facial Plastic and Reconstructive Surgery and to be certified with the American Board of Facial and Plastic and Reconstructive Surgery. He is a Fellow of the Royal College of Physicians and Surgeons and is also Board Certified by the American Board of Otolaryngology - Head and Neck Surgery.

Dr. Bartlett belongs to numerous Canadian and American medical associations, has co-authored several scientific publications, and has organized and lectured at facial plastic surgery courses for other surgeons. He also has an academic appointment in the Department of Surgery at the University of British Columbia where he is involved with teaching facial plastic surgery to residents in training.
