The Dorset Fertility service does not offer egg donation however we have regularly helped in the monitoring of egg recipients, ensuring that their womb lining (endometrium) is of an appropriate thickness for that part of the treatment cycle. There is a wealth of experience at Dorset Fertility and therefore most units are happy for their egg recipients to have some scans there to avoid the necessity of a long journey to the treating centre for monitoring. Decisions regarding timing and change of medication will always rest with the treating centre. Usually the scans can only be done on a private basis for a modest fee.

ICSI - Intracytoplasmic Sperm Injection

ICSI stands for Intra Cytoplasmic Sperm Injection. The technique was developed in 1992. ICSI is a modification of the IVF procedure whereby each egg is injected with a single sperm to fertilise it. The embryo can then be replaced into the uterus in the hope that a pregnancy may result.
 

IUI - Intrauterine Insemination

Intrauterine insemination (IUI) is one of several methods of assisted conception used in the treatment of cases when there is no obvious cause for the couple’s infertility. IUI is one of the simpler methods of assisted conception, which might be considered in couples thought suitable. IUI will usually be only considered in couples whose first investigations have failed to detect a specific cause of infertility and who have been trying for a baby for at least two years and where the female is under 35 years of age. Tests required prior to being considered for IUI will include blood tests to assess hormone levels and ovulation; patent fallopian tubes either by an hysterosalpingogram (HSG) or by laparoscopy and dye test for women, and a normal semen analysis for the men. The objective of IUI is to introduce a quantity of semen into the female partner’s uterus (womb), thereby encouraging fertilisation. The technique allows prepared sperms to pass beyond the cervix into the uterus using a fine IUI catheter.
 

IVF - In Vitro Fertilisation

To increase the chances of success of IVF the ovaries are stimulated with drugs to produce multiple egg development. This may result in an excess of eggs and embryos. Replacing more than one may result in a multiple birth. There are increased pregnancy risks from multiple pregnancy. For this reason only the best one or two embryos are transferred into the uterus.

Biography:

Julian Pampiglione qualified at the London Hospital Medical College in 1980. He first became interested in fertility matters in 1985. In 1986 he undertook a fertility research fellowship at Kings College Hospital. He has published widely on IVF and fertility and was a member of the Kings group led by John Parsons that pioneered ultrasound directed egg collection in the UK.

His postgraduate doctoral MD thesis was on male infertility. He has had experience of working in assisted conception at the Hallam Medical Centre, Bourne Hall, The London Women’s Clinic, Wessex Fertility, and the Winterbourne IVF Unit. He helped set up units at the Singapore General Hospital and Thompson Medical Centre, Singapore.

Over the last 20 years he has initiated and developed a comprehensive fertility service in East Dorset and was based at the Royal Bournemouth Hospital until 2011. Jointly with Michael Dooley in West Dorset he has established a comprehensive system of fertility services and succeeded in establishing NHS funding in Dorset in 1993

Julian managed the Early Pregnancy Unit and the recurrent miscarriage service for East Dorset along with the setting up of a satellite IVF programme in Dorset in 1996. His patients have achieved successful pregnancy rates both when satelliting with the Winterbourne Hospital from 1997, (37-44%) and Wessex Fertility (2007-10 43%, 50% in the under 35 year olds).
