5 50 Cataract Surgery

Just back home from Prague. I had livelong astigmatism and had recently developed cataracts. I was afraid of not being able to drive so after considering a few quotes I settled for Refrakcni Centrum. I m amazed that I did because I would say it easily has to be among the top clinics for eye surgery.

Dr Novak is nothing short of a magician and the care team is second to none. I wasn't anxious or nervous but I didn't expect it to be so comfortable. Absolutely pain free. The cost was less than a third of what I would have paid here in Scotland. So if you have such eye problems and are considering surgery contact this clinic. They are superb as are the results. Couldn't wish for better.

I have had eye surgery this week to replace my lenses for both eyes, with the Refrakcni Centrum in Prague. The staff there are all professional and charming. Dr Novac explained the choices and processes for my treatment very clearly, and we agreed a treatment plan together. So far the treatment has been trouble-free and my sight is improving day-by-day. It is only a few days since my final operation, so if things either continue to progress or do not, I will update this review.

It feels like I'm experiencing life anew

To be perfectly frank, I've considered Lasik surgery as an unnecessary luxury. Turned out I underestimated by far how much of an impact perfect vision brings. From small things like shaving, through everyday life freedom, up to watching the horizon line from an aeroplane's window - it feels like I'm experiencing life anew. Not only does it not feel like an overindulgence no more, it feels like I ended up with a once-in-a-lifetime barging. It would be worth it on it's own, getting it done at RCP only made it so much better.

If someone tells you getting eye surgery is not scary, they are probably laying. Course it is. The thing with fear is, though, that it can be managed and mitigated, and staff at RCP manages that brilliantly. All the way from the primary examination, throughout the surgery itself, everyone made sure I was well informed and conscious, to a point where I started asking myself why my previous opticians didn't mention this or that. By the time the surgery came, I felt like making sure I'm comfortable is the hard part of the process, procedure itself being almost an afterthought - and it was a breeze. I lack words to express my gratitude to the staff for their professionalism, care and warmth - all the way from reception desk, nurses, to the doctors and surgeon. If anything, how smooth the surgery and healing process goes is a bit disappointing, since I have no reason to visit RCP again.

If you are someone like me - on the edge, wondering if going for LASIK is a smart choice, since you can live perfectly fine with glasses? It's worth it. It's doubly worth it at RCP.

Everything is so bright, we have to wear sunglasses

My husband and I both decided to have the Refractive Lens Exchange operation in Prague, because the cost of the same operation in UK would have been impossible for us to afford. I struggled with close up reading, and hubby with distance vision. I chose the Refractive Center Prague because they had good reviews and were very responsive when I sent emails asking questions.

You have to have had a medical check-up stating you are ok to undergo local anaesthetic - we had this done at their partner clinic Next Clinic in Prague because we discovered that our GP was not prepared to do it. When you go to the Next Clinic remember to bring your passport with you, the entrance to the clinic is next to the one that says Next (go figure, maybe that’s why it’s called Next). They did blood and urine tests, an EKG and a cardiologist checks you over. The results are emailed directly to the Refractive Center the next day.

The Refractive Center in Prague is easy to find, a number of trams stop about a block away.
The receptionist didn't speak English but she was laughing and friendly, we were given new masks to wear (everywhere in Prague requires you to wear a mask due to Covid19).

The surgeon Petr came over and said hello, and then various tests ensued. There was the usual peering into lenses while a puff of air is blown into eye, then we each went through to Petr where he explained exactly what would happen regarding the surgery. He conducted more sight tests and asked us about activities, hobbies, the amount of time we spend driving, computer work etc. Then we sat while pupil dilation drops were applied. After about half an hour we went back to Petr’s office where we had to look at an intense bright light, and then sat with eyes closed to recover.

Once Petr has checked all the results and is happy, he gives the go ahead for the operation to take place, which in our case was later that afternoon. He showed us the lenses, in large clear plastic circles with rings around them. That will sit in a dome shape in eye. I think!

All these checks take some time – in our case the entire morning, but it was good to know they were being so thorough.

The hospital where the operation takes place is on the other side of the city, but it’s simple to get there by tram or uber.

When we arrived at the hospital a ray of sunshine in the form of smiling bubbly Jana (Mrs Surgeon) came to fetch us to go upstairs to the preparation room. With much laughter she tells us to relax in the room, it is all ours to use with an en suite in case we need it. She takes our blood pressure and offers us each a calming tablet in case we are nervous. We remove all jewellery and clothes and put on paper scrubs with a lovely hair net hat!

We sat in our lovely blue paper scrubs whilst Jana applied eye drops several times over the next half hour.
Then she took me to surgery, the nurses helped me climb up on the bed and lie down. Petr comes over and gives me a round rubber ring to hold, I tell him I would prefer a doughnut. He and the nurses laugh. The next 5 minutes are spent chatting about favourite foods, then he starts the surgery.
I felt nothing, could just see shapes and at one point tiny bright light speckles on a black background. Then it is all over, not even 10 minutes I don't think. Jana appears again and walks me back to the room where I get dressed and then she leads hubby away for his turn.

The operation was so quick and painless, and we received really excellent treatment. Jana tells me Petr is very experienced, over 20000 operations - so we were in good hands.

We had to wear an eye patch back to the apartment and put eye drops in every hour and wear a new eye patch for sleep to prevent us rubbing our eyes.

The next day we were out and about, as usual, my eye felt a bit uncomfortable like I had something in it. But not enough to bother me. I noticed a colour difference between my old eye and my new eye, green looked blue-ish, and white was really bright. I also found everything a bit blurry with my new eye. But, the big thing for me was I could suddenly read menus, read my phone, all without glasses! That was on the next day. Not totally clear, but I could read. Letters appeared to have a blurred outline around them, like a large 3D shadow.

2 days later we returned for a check-up. More looking at hot air balloons, puffs of air and bright lights. All good - we can ask Petr any questions later that afternoon when we go for the second operation.

The second surgery went as smoothly as the first, again it was really quick and painless. We didn't have any questions, although Jana gave us contact numbers and a card with a barcode on it which when scanned will show exactly what lens we have in our eyes.

After 2 weeks I can read and hubby can drive without glasses, although our near vision is still a bit blurry. I also see halos around bright lights, this should diminish in time. I was wondering when the blurry outline on close-up letters would go away, so I emailed Barbora at the clinic and she assured me it is quite normal as the brain has to adjust to the new lens. It should normalise in 4 to 6 weeks.

Everything is so bright, we have to wear sunglasses!

We're both really pleased we had this done, the difference is incredible. I was very impressed by the treatment and communication from all the staff at Refractive Center Prague, they are very professional and approachable. They are still in touch with me via email, and I feel comfortable knowing they are easily accessible to answer my questions.

Absolutely fantastic

Absolutely fantastic. Both myself and my husband are delighted with the results. 1 eye operated on Wednesday and by Thursday afternoon what a difference 2nd eye done on Friday flew home today and eyesight is brilliant.

I had laser eye surgery, my prescription was -6 in both eyes which meant that I was severely short-sighted. I reviewed this clinic and got in touch to ask some initial questions which they answered promptly and clearly. I then booked a date and flew to Prague to have my surgery done.

On the day of the surgery, everything went as discussed. All the staff were pleasant, spoke English and the building is modern and well maintained. The surgery went well, it took about 10 minutes for both eyes and I now can see as if I had my glasses on which is a perfect result for me. I would recommend this clinic.

My experience was very good from the reception staff to the surgeon. Everything was explained clearly; the risks, the benefits and I was armed with sufficient information to make an informed decision which ultimately resulted in a successful treatment with me now having excellent vision.

I had laser eye surgery and the results are great, I can see now as if I had my glasses on

The treatment I had was for both eyes I was -6.50 in both eyes and was very shortsighted. I had laser eye surgery and the results are great, I can see now as if I had my glasses on.

Everything was explained clearly as due to my sight being so bad and my age it was unclear how much of an improvement there would be. However, I still went ahead as I knew there would be an improvement this was all explained very clearly.

As explained during the surgery it is painless as your eyes are anaesthetised, after there was some discomfort but not pain. However, that went after 3 to 4 hours and now everything fine. Just taking eye drops, eyes in the morning take a few minutes to adjust but it's still early days as I am only two weeks after surgery. I would highly recommend.

The people at the clinic are very professional and it is a modern clean building. Everything was explained clearly so I knew what my options were. Polite and friendly.

My experience from the first day was excellent

5 50 Cataract Surgery

I had two treatments. The first to replace the lenses, as one of them, started showing cataract changes. Also, I had severe astigmatism in both eyes, with severe vision problems. As well as a problem for near vision.

All my vision problems confined to the lens was corrected by having a new multifocal lens implanted. While astigmatism which is an uneven cornea was corrected by special laser treatment. Everything went well.

My experience from the first day was excellent. The arrangements and help to visit Prague were very friendly. Excellent care from Dr. Pter Novac the ophthalmic surgeon as everything was done professionally, care before the operation, during and after. As I am myself a medical doctor, I do strongly recommend any eye condition. I found to be the best value for money.

Specialised Eye Examination Prior to Laser Eye Surgery (2 hours)

The examination includes complete anamnesis as well as eyesight anamnesis, establishing the exact dioptre error (manifest refraction), a detailed examination of the frontal segment of the eye, measuring of the intraocular pressure as well as the Schirmer test. Examination of the rear of the eye follows. To carry out a thorough examination of the rear of the eye it is essential to dilate the pupils by administering special eye drops. As a result, we can also examine the periphery of the retina (this is important, especially in patients with myopia) and measure the values of the so-called cycloplegic refraction (as these values may differ from the values obtained when the eye is not dilated). This process minimises the risk of miscalculation.

A part of the initial examination is also a check-up using the PENTACAM and iDESIGN devices. These provide readings essential for the surgeon to determine the suitability of the patient for laser eye surgery.

We always carry out the initial eye examination prior to laser eye surgery on a different day than the actual surgery. This is the only way to ensure optimal post-surgery results.

iDESIGN iLASIK

iDESIGN iLASIK is the safest and most precise laser eye surgery method used for the removal of short-sightedness, far-sightedness and astigmatism. It uses technology certified by NASA, meaning that it complies with the strictest safety criteria.

iDESIGN iLASIK uses advanced and tested technology throughout the procedure and as such ensures a 100% tailor-made procedure. It is this individualisation, in particular, that differentiates this method.

During the initial examination, part of which is the iDESIGN device, we immediately, in a single instant, acquire complex results of the five basic parameters of the eye. This data is as unique as a fingerprint. Based on these results, we are in a position to propose a tailor-made and transfer this plan into the memory of the VISX STAR S4 I laser. The laser then carries out the required changes to the curvature of the cornea with absolute precision.

The surgery is carried out under local anaesthesia applied through eye drops. Throughout the surgery, the patient is fully conscious, however, registers no pain. The entire procedure lasts only a few minutes. The recuperation is very quick and the return to improved vision functions is fast.

Who is this method meant for?

Especially patients suffering from one of the following:

  • Short-sightedness (low, medium and high)
  • Far-sightedness (up to +4,0 dioptres)

Intrastromal Corneal Ring Segments Implantation (Femtolaser)

ICRS may be used in the treatment of short-sightedness (from −1.00 to −3.00 dioptres) in patients with keratoconus and with

ecstasy after refractive laser procedures.

Indication for ICRS implantation is the transparency of the central part of the cornea and intolerance to wearing contact lenses. The first step is the plan of the procedure. A thorough analysis of the patient’s data acquired in the initial examination at the corneal advisory centre is carried out. An optimal type of ICRS is determined (shape, curvature and thickness of the segment, how many degrees of the circular segment, one or two segments) so that optimal post-surgery results may be achieved.

FemtoLASIK (LASIK with IntraLase)

The FemtoLASIK method is a superb modern technology of removal dioptres using two latest lasers: femtosecond laser iFS and excimer laser VISX STAR S4 IR, both from Johnson & Johnson, an American company.

The ultra-fast femtosecond laser is used (using the principle of photo-disruption) to create a thin lamella on the cornea. The Laser beam is focused into one single point where energy is concentrated, and the tissue of the cornea immediately turns to gas (it literally evaporates). The lamella created in this way is more perfect and smoother than when an oscillating blade is used. The lamella in the cornea is then lifted to the side and using the effect of the excimer laser, the cornea is re-shaped into the required shape and curvature as a result of which the dioptre error is corrected. Once done, the lamella of the cornea is returned to its original position and the surgery is completed.

We recommend this method specifically to patients with the following:

  • Short-sightedness (lower, medium as well as higher)
  • Far-sightedness (from +4,0 dioptres on)
  • Relatively symmetrical astigmatism (up to +/−4,5 dioptres max.)
  • Uncomplicated refractive error without significant asymmetry

PRK - Photorefractive Keratectomy

The PRK method is the oldest refractive laser technique of all but, in some cases, it is still the best option available. In essence, this is correcting the curvature of the cornea, once the epithelium has been mechanically removed. Once the surface of the cornea has been desensitised using anaesthetic eye drops, and the epithelium has been removed (using a surgical instrument or a little brush) the laser procedure is carried out within the surface layers of the stroma.

Because the epithelium forms a sort of natural shield, without which the eye becomes extremely vulnerable or even painful, the cornea must be covered using a contact lens after the procedure. The covering layer of the epithelium is regenerated relatively fast, over the course of the following few days, but even so, the healing process is often accompanied by pain and un-sharp vision. The unpleasant feelings associated with the surgery usually cease within the next 3 to 4 days. The definite healing of the cornea and complete vision stabilisation is largely individual but usually, this happens within the next few weeks or months. Optimal vision may be expected within one month or a little longer. This is why (unlike procedures of the LASIK type) we always carry out the surgery of one eye first and the surgery of the other eye is recommended to be completed within the next one to six months after the first one.

We recommend this procedure to patients with one of the following:

  • Short-sightedness and astigmatism (low dioptre error of up to −2,0 dioptres)
  • Patients with cornea that is too thin
  • As a re-operation having undergone another procedure previously (with the possibility of an individualised procedure)

PTK - Phototherapeutic Keratectomy

This procedure is usually carried out under topical (surface) anaesthesia. Once the epithelium is removed, the patient actively co-operates by having to follow a fixed point. Only with eccentric or peripheral lesions, the surgeon must manually turn the head or the eye of the patient or possibly use a joystick to place the laser beam in the desired spot.

PTK is carried out using an excimer laser (193 nm) and uses the principle of photoablation – the interruption of the connection between molecules. A single pulse of the laser removes exactly 0.25 micrometres of corneal tissue. In this way, we have the option to plan ahead the exact desired change of the shape and ensure that the treated surface will be smooth and regular – this will improve the visual result. In the case of treatment of Recurrent Corneal Erosion, the aim is to reach a diametrically neutral result – meaning keeping the original dioptres. In such a case, the depth of the ablation on the entire surface, in the range of 6–9 millimetres, is only some 5–6 µm.

If the cornea is otherwise healthy, it is possible to combine the therapeutic effect of PTK also with improved refraction. In such a case we are talking about carrying out PRK as PTK. In this way, it is possible to eliminate myopia, possibly also myopia combined with astigmatism.

After the procedure, we cover the surface of the cornea with a contact lens so that the pain is minimised. The pain following this type of surgery may be very intense in the first 2–3 days post-surgery. We keep the contact lens in the eye for the duration of 2–4 weeks. Cataract Surgery

During cataract surgery, the surgeon first removes the cloudy natural lens from the eye and in most cases replaces it with a clear artificial intraocular lens. The surgery is usually ambulatory and does not require hospitalisation. Most modern cataract surgeries are carried out using a high-frequency ultrasound device that divides the cloudy lens into minuscule pieces. These fragments are subsequently gently suctioned out of the eye.

This method, known as 

phacoemulsification (or simply phaco), is performed using much smaller incisions than were used in previous surgical techniques for the elimination of a cataract. These incisions enable faster healing and reduce the risk of complications such as for example the detachment of the retina. 

Once all the bits of the affected lens are removed from within the eye, the surgeon inputs the clear new artificial intraocular lens. The surgeon then secures the lens in the area beyond the iris and pupil – into the exact same spot where your natural lens was original.

The surgeon then completes the procedure by closing the incisions in the eye (sutures may or may not be used) and a protective bandage is placed over the eye.

Specialised Eye Examination Prior to Intraocular Eye Surgery (3 hours)

 Before undergoing the procedure, it is required to first have a thorough and entirely painless complex eye examination. It includes complete anamnesis of the patient’s eyesight, establishing the exact dioptre error, detailed examination of the frontal segment of the eye, measuring of the intraocular pressure as well as the tear test. It further includes an examination of the intraocular lens as well as the rear segment of the eye.

It is essential to administer special eye drops to dilate the pupil prior to this examination. In this way, it is possible to discover any dislocation of the lens, the loosening of the suspensory apparatus, pathology of the pupil casing or any anomaly of the vitreous humour. It further enables us to examine the retina including the periphery of the central visual field and the optic nerve.

Examination using the unique PENTACAM device is part of the initial examination. The AL-SCAN device enables us to carry out a biometric examination of the eye with a subsequent calculation of the dioptre value of the implanted lens. Should it be necessary, we will carry out other specialised examinations such as the examination of the field of vision, strabismus analysis, OCT etc.

Based on these results, the surgeon will establish whether intraocular eye surgery/cataract surgery is suitable. Having done all of the above and having taken into consideration the patient’s expectations and the specialist’s recommendations, it is possible to determine the ideal type of intraocular lens.

Preventive Eye Examination (1 hour 30 mins)

Our complex eye examination takes about 90 to 120 minutes. It is entirely painless. It involves establishing complete eye anamnesis, determining exact dioptre error, a thorough examination of the frontal segment of the eye, measuring the intraocular pressure as well as the Schirmer test.

In addition, during the examination, we also check the rear of the eye whereby it is necessary to administer special eye drops that dilate the pupil. We perform the examination of the cornea and the entire frontal segment of the eye using PENTACAM.

Examination by a Corneal Disease (1 hour 30 mins)

We offer our patients suffering from corneal diseases an examination at our specialised corneal disease examination centre. This examination centre boasts the unique PENTACAM device that enables early diagnosis of keratoconus (the progressive thinning and deviation of the cornea) as well as other ailments affecting the cornea including post-traumatic conditions.

The initial examination at our specialised corneal disease centre takes 1.5 to 2 hours and it is entirely painless. It includes complete anamnesis of the patient’s eyesight, establishing the exact dioptre error, detailed examination of the frontal segment of the eye, measuring of the intraocular pressure as well as the tear test. Examination of the rear segment of the eye follows. To do this, it is essential to dilate the pupils using special eye drops.

The consultant is Dr Petr Novák, who is involved with this specialised segment of ophthalmology as part of his clinical practice. As such it is possible to follow up on other specialised examinations as well as operative solutions including the transplantation of the cornea.

Implantation of Phakic lens Visian ICL

This surgical procedure is meant primarily for the correction of a greater degree of short-sightedness and far-sightedness. It is used in cases where a laser surgical procedure would be ineffective or risky. Today, it is also possible, in some cases, to resolve the problem of presbyopia using a phakic multifocal lens.

This is an intraocular surgical procedure that we perform only in cases whereby the patient fulfils relatively strict pre-surgery criteria (sufficient density and quality of endothelial cells in the cornea, suitable anatomical requirements, and normal intraocular pressure).

During the procedure, we place the implant (phakic lens) inside the eye, into the optical axis while at the same time keeping the natural lens (as opposed to the refractive extraction of the lens). Combining the implanted lens together with the natural lens we achieve the required correction of the dioptre error. At the same time, we maintain accommodation (focusing) of the natural human lens in case of myopia and hyper-myopia in younger individuals or we replace accommodation when resolving presbyopia.

Probably the most often used phakic implant is a lens implanted behind the iris, once that copies the frontal surface of the natural lens. This type of lens is known as intraocular contact (Implantable Contact Lens – ICL) and is the only method used in case of correcting presbyopia.

Today, this procedure is most commonly performed under local anesthesia once the eye has been desensitised using eye drops. It is also possible to perform this surgery under complete anesthesia. The patient is required to rest after the surgery and to apply the prescribed eye drops. The surgery of the other eye is performed 2 days to two weeks after the first eye depending on the type of implant used and the individual healing ability.

PRELEX (Presbyopic Lens Exchange)

The term PRELEX is used to describe a surgical method whereby we replace the natural lens with an artificial multifocal or EDOF (Extended Depth of Focus) lens.

The surgery takes place as part of an outpatient clinic and the operated eye is desensitised using eye drops. At the same time medication dilating the pupil is administered, so that the lens positioned behind the colourful iris, becomes accessible for the surgical procedure. In patients that are very nervous or anxious, it is possible to administer a calming pill. The procedure itself takes about 15 minutes. In some clinics, both eyes are operated on at the same time. At our clinic, we opt to only operate one eye at a time for several reasons. The other eye is operated on within 2 days from the first surgery.

The main surgical incision is done through the cornea. Depending on the type of implant, the incision is most commonly 2 to 2.5 mm wide and because of its self-sealing property, there is no need for sutures.
