"I could never breathe very well due to a severe deviated septum and it started to affect my life pretty badly. I was uncomfortable all the time and I needed to fix it ASAP. Thankfully I got in touch with Murat and the whole process was easy, painless and I'm now recovering well. My stents came out today and the feeling of being able to breathe freely is incredible. Truly life-changing :) If you have a deviated septum I cannot recommend getting a septoplasty and turbinate reduction enough. I should have done this 20 years ago.

"I had a Rhinoplasty to fix some deviations in my nose, to correct an overly projected jump and deproject the tip of my nose. Initially I started looking for surgeons in Greece (my hometown) however, I found that the ENT surgeons there where not performing many rhinoplasties. I started to look in Turkey, because from my understanding it is a very serious country with it's regulations, and the doctors are well practiced and educated. I came across Dr. Murat, and his websites were fully documented with his work, how he practices, advice and the details of his operations. I found it very important that his website is more of a 'lifestyle blog', rather than a generic plastic surgery website, as I truly felt that this man adores what he does. His results on his blog are well-balanced, natural and beautiful. But there are more if you search well. My father, who used to also be a surgeon, researched with me, and he was very impressed with Dr. Murat - he insisted I visit him and no one else. I asked him 'why are you so sure?', he told me 'when a doctor loves what he does, this is the only thing you should be looking for'. "I have a long history of ear problems and in fact I had a surgery to replace the small bone that transmits the sound to the inner ear that was long gone due to continous infection since childhood. Needless to say, the only ENT I trust with my ears is the Dr. who performed the surgery and he is the only one I allow even to remove wax from my ears. My ear drum ruptured during the last part of a long flight to Istanbul. I knew I had to see an ENT immediatley and I started googling "best ENT in Istanbul." I trusted the excellent reviews about Dr. Murat Enoz and I called his office on a Friday. His office promptly replied and asked me if I was available in a couple of hours after my call. I replied with a request to go on Saturday morning. Dr. Murat Enoz was there waiting for me in his tastefully decorated and welcoming office. I confess, I was a bit nervous but Dr Enoz was reassuring and calmly checked the injured ear, removed wax in the other ear, checked my nose and throat thoroughly. I could see the whole process done on a video by the examination chairs I have never seen such sophisticated technology in my ENT clinic at Seattle's well renowned Overlake hospital. Dr, Murat completed his examination and prescribed me the needed medications, called the pharmacy and have them deliver at his office while I was waiting in his cozy leaving room. Tympanoplasty

A tympanoplasty is a surgical procedure which generally used to repair a ruptured eardrum but sometimes involves the repair of the ossicles in middle ear. Otosclerosis Treatment

Otosclerosis, is the most common cause of stapedial ankylosis (or stapedial fixation). Its cause is unknown and resulting conductive hearing loss.

Different ENT surgeons use a variety of terms for partial stapedectomy such as stapedotomy, stapes replacement, teflon piston procedure, small fenestration technique, etc.

Audiological investigations which are, pure tone odiogram, speech odiograms, impedance audiometry, stapedial reflex tests, tuning work tests must be done. Radiological investigation includes high resolution CT scan and Schueller's graphy to acces the congenital inner ear anomalies and to demonstrate the cochlear otosclerosis.

Stapedectomy may be carried out either under local anesthesia with sedation or general anesthesia.

After surgery, if you have a head dressing, remove it on the second post operative day. A cotton ball in the ear opening will be found when the dressing is removed. Please remove the cotton plug on the second day after surgery. You may see further packing in your ear canal. Leave this until your post-op visit.

Septoplasty

The nasal septum separates the nasal cavity into the two nostrils and it contain the bone and cartilage. The cartilage is called the quadrangular cartilage and the bones comprising the septum include the maxillary crest, vomer and the perpendicular plate of the ethmoid.Nasal septum deviation (deviated nasal septum) is defined as displacement of the nasal septum. It is an abnormal condition in which the top of the cartilaginous ridge leans to the left or the right, causing obstruction of the affected nasal passage. It, can lead to contraction of the air passages of the nose. Although at different levels of septum daviations are available in many people.

Most septal deviations result in no symptoms. Some septal deformities, however, may cause the following signs and symptoms: - infections of the sinus- sleep apnea, snoring- repetitive sneezing- facial pain- nosebleeds- difficulty with breathing-mild to severe loss of the ability to smell

A simple majority is sufficient for diagnosis of ear, nose and throat examination. Endoscopic examination and computed tomography allows for a more detailed assessment.

Medical treatment (such as such as decongestants, antihistamines, and nasal spray) relieves symptoms, but does not correct the underlying condition. Non-medical relief can also be obtained using nasal strips. A cure to symptoms related to septal deviations is available in the form of a minor surgical procedure known as a septoplasty or submucous resection (SMR). The septoplasty operation is performed quickly (lasts roughly 1 hour) and does not result in any cosmetic alteration or external scars. Recovery from the procedure may take anywhere from 2 days to 4 weeks to heal completely.

Sinus Surgery
Currently Endoscopic Sinus Surgery and Balloon Sinuplasty are more favorable treatment techniques for chronic sinusitis. Caldwell-Luc operation is defined before the endoscopic sinus surgery to relieve chronic sinusitis by improving the drainage of the maxillary sinus, one of the cavities beneath the eye. The maxillary sinus is entered through the upper jaw above one of the second molar teeth. A window is created to connect the maxillary sinus with the nose, thus improving drainage.

FESS - Functional Endoscopic Sinus Surgery

Functional endoscopic sinus surgery (FESS) is the primary safe approach used currently for the surgical treatment of chronic sinusitis.

This procedure is a minimally invasive, safe and it can be performed entirely through the nostrils, leaving no external scars. There is little swelling and only mild discomfort. It can be combined with other nasal operations (septoplasty, turbinate radiofrequency etc.).

A recently developed alternative to functional endoscopic sinus surgery is balloon sinuplasty. This technique uses balloon catheters to dilate the maxillary, frontal, and sphenoid natural ostia without bone or soft-tissue removal. Reports show persistent patient symptom improvement and sinus ostia patency.

The most common indication for endoscopic sinus surgery is “chronic rhinosinusitis”.
The most common indications for endoscopic sinus surgery are as follows:
• Chronic sinusitis refractory to medical treatment

• Recurrent sinusitis

• Nasal polyposis

• Antrochoanal polyps

• Sinus mucoceles

• Excision of selected tumors

• Cerebrospinal fluid (CSF) leak closure

• Orbital decompression (eg, Graves ophthalmopathy)

• Optic nerve decompression

• Dacryocystorhinostomy (DCR)

• Choanal atresia repair

• Foreign body removal

• Epistaxis control

There are probably no absolute contraindications to endoscopic sinus surgery.

The most common relative contraindications for endoscopic sinus surgery are as follows:

• Absence of specific osteomeatal complex abnormalities.

• Oesteomyelitis involving the sinuses.

• Frontal sinus disease with stenosed internal ostium.

• Threatened intracranial and / or intracerebral complications.

• Inaccessible lateral frontal sinus disease.

Smoking is also consider as a relative contraindication to endoscopic sinus surgery.

Tonsils and Adenoids

Tonsils and adenoids are the elements of Waldeyer's lymphatic ring which settled in oral and nasopharyngeal region. Both are play role in immunity against airborne microorganisms which inhaled through the mouth and nose.

The tonsils are located on both sides of the back of the throat. The other names of adenoid are pharyngeal tonsil and nasopharyngeal tonsil. This lymphatic tissue is situated posterior of the nasal cavity, the roof of the nasopharyngeal area.

The eustachian tubes is opened to the nasal cavity in this region.

Why Tonsils and Adenoid Enlarged?

Tonsils and adenoid can grow depending on the recurrent infections and genetic causes. Living in crowded environments (such as kindergarten), exposure to cigarette smoke, atopy can be facilitated to lymphatic tissue growth. Although extremely rare, cancer sometimes causes enlarged tonsils or adenoids.

Symptoms of Enlarged Tonsils and Adenoid

The tonsils and adenoid are settled on the airways. Most enlarged tonsils and adenoids cause no symptoms. However, when they grow up in addition to breathing problems, ear and sinus related problems also can be seen.
Enlarged tonsils and adenoids may cause the following problems:
- Chronic ear infections and hearing loss: These problems result from blockage of the eustachian tube and fluid accumulation in the middle ear.

Lingual Frenectomy

Lingual frenulum is a structure that normally located between the normally floor of the mouth with the tongue. If it become short and thick as congenital oral anomaly it is called as Tongue tie or Ankyloglossia.

This structure can be varying degrees of shortness and thickness. It may range in thin short mucosal membrane to thick fibrotic band. Ankyloglossia can affect the feeding, speech and oral hygiene.

The symptoms of tongue-tie can include that:

- Acting irritable or fussy, even after feeding
- Difficulty creating or keeping suction on the nipple. The infant may become tired in 1 or 2 minutes, or fall asleep before eating enough.
- Poor weight gain or weight loss
- Problems latching onto the nipple. The infant may just chew on the nipple instead.
- Halitosis (bad breath) may be present, due to caries and accumulated food debris.

Difficulties can include creating sounds that need the tongue or tongue tip to (source: Tongue-tie | Better Health Channel):

- touch the roof of the mouth, to pronounce sounds such as ‘t, d, n, l, s, z’
- arch off the floor of the mouth, to pronounce sounds such as ‘r’.

Structural changes on tongue may occur over time ??such as:

- Heart-shaped tongue tip
- Square or round tongue tip

Most breastfeeding problems can be managed easily and the mother has received at least 2 to 3 days of support from a breastfeeding (lactation) specialist.

Tongue tie surgery (sometimes referred to as "frenectomy", "frenulectomy", "frenotomy" or "frenuloplasty") can be performed easily in a few months old babies with office conditions.

Surgery for more severe cases and greater babies is done in a hospital operating room. A general anaesthesia can be preferred aspecially after 6 months of age. Rhinoplasty

Rhinoplasty or nose job is a surgical prcedure to correcting and reconstructing with restoring the functions of the nose. The main objective of rhinoplasty is creating a functional, aesthetic and facially proportionate nose.

Rhinoplasty can be performed using general or local anesthesia. It is usually done as an outpatient procedure but sometimes requires one day staying in the hospital.

Two methods used for rhinoplasty:

1 - closed rhinoplasty
2 - open rhinoplasty

In the closed technique rhinoplasty, postoperative edema is expected to be less because of less tissue trauma, anatomical structures evaluation is more difficult from the open technique.

In the open technique rhinoplasty, nasal skin is dissected, the nasal cartilages visible, anatomical structures are more easily seen. The skin of the nose is separated from the nasal bone and cartilages.
