Plastic Surgeon Consultation
When you contact the Clinic we will arrange a consultation with Mr James Small, Consultant Plastic Surgeon. Your consultation will be private and confidential. You will be afforded all of the time that you need to fully understand the procedure you are contemplating. The initial consultation will consist of a review of your medical history, clinical examination and a full discussion of your concerns and expectations. The nature of any proposed treatment will be discussed in detail including the procedure, recovery time and any risks the procedure might entail. Where appropriate photographs will be taken to assist the discussion and written information is available about individual operations. With regard to major or cosmetic procedures the consultation will take place well in advance of your proposed treatment, giving you ample time to make the right decision. With regard to minor skin lesions such as moles and other skin lesions it is often possible to have the lesion excised in conjunction with the initial consultation thus reducing the number of visits to the Clinic. Your consultation will be with the surgeon who will carry out your operation. Post-operative care will be carried out by your surgeon., You will be reviewed at the Clinic until your recovery is complete and you are satisfied with the result of your operation.
Rhinoplasty
Rhinoplasty (Nose Reshaping) This procedure reshapes, reduces and reconstructs the nose. The nose is made up of bone (bridge) and cartilage (tip). Unsightly deformities can take many forms including :• a hump on the bridge • an over-sized tip • a pointed tip • a nose that is too long (crooked) (common)• a short nose (uncommon) • a deviated nose  Many patients tell me that they have been unhappy about their nose for many years indicating that they think long and hard before deciding to explore or undergo surgery. Following rhinoplasty many patients say they regretted not having had the operation carried out years earlier. Age Group:A rhinoplasty is rarely carried out before the individual is fully grown. Beyond late teens a rhinoplasty can be carried out at any age in adulthood. There is no strict upper limit for rhinoplasty in terms of age. The Procedure:The majority of rhinoplasties are carried out under sedation and local anaesthetic as day cases. 10% require a general anaesthetic but can be treated as day case patients and only 5% are hospitalised overnight. An external splint is applied to the nose to maintain the improved shape. The splint is made of light material (steri-strips) and it is easily removed at the appropriate time – 7-10 days following the procedure. Operation Time: A reconstructive rhinoplasty, i.e. previous and failed rhinoplasty(s) or previous severe trauma, may take two to two and a half hours and might entail cartilage grafts or bone grafts. Recovery Time:If the nasal bone has been fractured the eyes will usually be bruised, otherwise bruising is remarkably slight or not at all. Bruising resolves in about seven days. The external splint is removed after seven days, by which stage you should be able to go out socially or return to work.
Eyelid surgery
The presence of loose, sagging skin (hooding), lines around the eyes (crows feet) and bulging fat under the eyes (bags) – characterise ageing of the eyes. The aim of surgery is to remove sagging skin, lines and bags thus restoring a more youthful, fresh appearance.Age Group:Who is suitable for cosmetic eyelid surgery ? The procedure can be carried out at any age but most candidates are over thirty.Anaesthetic:Most procedures can be carried out under sedation and local anaesthesia. General anaesthesia is optional but rarely necessary.The Procedure:• .• The incisions: in the upper eyelid a surgical incision is made in the eyelid crease and in the lower eyelid an incision is made close to the eyelashes. The incisions frequently must extend out into the crow’s feet / laughter lines area around the eye.• Redundant skin and fat and occasionally muscle are removed.• Bags under the eyes are caused by protruding fat. During surgery this fat is returned to the peri-orbital fat from where it came originally. The supporting layer (orbital septum) is repaired. This is equivalent to repairing a hernia.• Usually the supporting ligament of the lower eyelid requires tightening (canthopexy) to prevent post operative drooping of the eyelid (ectropion).• On completion of the procedure sutures are inserted above and below the eye and steri-strips are applied.Hospitalisation:Blepharoplasty is carried out as a day case procedures. The procedure takes between one and a half to two hours. Overnight admission is optional but rarely necessary.Recovery and Aftercare:The level of discomfort is slight. The eyelids particularly the lower eyelid tend to bruise easily. Vision is unaffected. Light pain relief may be taken. Lubrication of the eye with ointment especially at night is very important. Arnica may also be used for bruising. The sutures are removed after about four days. Bruising usually takes two to three weeks to resolve.
Gynecomastia
Gynaecomastia (Male Breast Reduction)This is a condition where female like breast development occurs in males. The condition is frequently disturbing to the individual causing him to seek professional advice and ultimately surgical correction. It is particularly prevalent in adolescents. Age Group:Gynaecomastia is particularly prevalent in adolescents. However it also occurs with relative frequency in adults and older men. Assessment:Assessment of the individual with gynaecomastia is based on two fundamentals:1. Is there an underlying condition ? In the vast majority of cases there will be no associated medical condition. A thorough medical history and clinical evaluation must be carried out. 2. In the majority of individuals a detailed assessment of the breast is the basis of a successful outcome. The distribution of glandular breast tissue and fat must be recorded. The significance between the two tissue types is that fat can be successfully removed by liposuction whereas the firmer glandular tissue cannot be suctioned. It must be removed by open surgical excision. Anaesthetic:Local anaesthetic and sedation. General anaesthesia is required in adolescents.Hospitalisation:The majority of patients undergoing surgery for gynaecomastia do not require overnight admission to hospital and they can therefore be treated as day case patients. The Procedure:This entails the reduction of:1. The fibrous glandular tissue which is usually concentrated in the centre of the breast behind the areola / nipple.2. The fat tissue which is distributed more widely throughout the breast.Operation Time:Surgery for gynaecomastia can take approximately one to one and a half hours. Aftercare:An elastic compression garment / vest is worn immediately following the surgery. It is worn for at least four weeks. Medications will include an antibiotic for five to seven days and pain relief.
Tummy Tuck
Abdominoplasty (Tummy Tuck)Introduction: This procedure is becoming increasingly popular. The majority of candidates are women of childbearing or middle-age with a history of one or more pregnancies. Significant weight gain or obesity followed by weight loss will often result in sagging abdominal skin. Men occasionally request an abdominoplasty and the procedure can be successfully performed in males as well as females. Age Group: The peak age group is therefore between thirty and fifty . Anaesthetic: The procedure is carried out under general anaesthesia and takes approximately two to three hours. Most patients stay in hospital for two days following the operation.  The Procedure: A tummy tuck is performed through a lower abdominal incision. The skin of the abdomen is dissected off the underlying muscles from the lower abdomen (pubic area) to the upper abdomen (ribcage). Dissolving sutures are used as opposed to staples. Staples tend to be more uncomfortable, they must be removed after about one week and they may leave marks in the skin.  Dressings are applied over the incisions and finally an elasticated pressure garment is worn for approximately one month. Two small plastic drains are used to ensure that fluid does not collect under the abdominal skin.

The timescale reflects the intricate nature of this procedure.
