There are a number of injectable, wrinkle-filling agents (fillers) available which are classified as: • Semi-permanent (temporary)• Permanent• Implants which are permanent but removable Ideally, a temporary wrinkle filler should not cause allergies, not move from its original placement, be safe, be injectable through a very fine needle to minimise discomfort, be undetectable once in place and be long lasting and be of minimal cost. At present no fillers satisfy all these criteria completely. Permanent injectable fillers have been used for many years particularly for the lower part of the face. It is currently considered by most medical practitioners including ourselves that permanent injectable fillers should not be used to treat facial lines and wrinkles because of possible long-term complications. Therefore, the Dublin Cosmetic Surgery Centre does not offer any permanent injectable fillers. Permanent implants have been in use for many years and many are now available for augmenting different parts of the face e.g. cheeks, chin, lips. They are made of inert materials to minimise tissue reaction. The commonest material to be used is silicone. A permanent implant differs from a filler in that it comes prepared as a solid material which can be carved/modified in size and shape and which will not react with human tissue. In addition once placed under the skin it can be readily removed. Thus an implant will be sited in a pocket under the skin whereas a filler once injected will dissipate (spread) into the subcutaneous tissue.
